1. Acknowledgment of Risks
I, the undersigned parent/legal guardian, understand that participation in physical play activities at the Street Play Event involves risks, including but not limited to:
- Trips, falls, bumps, or collisions
- Injuries caused by other participants
- Exposure to outdoor conditions
I voluntarily allow my child to participate, understanding these risks.
2. Waiver & Release
In consideration of my child’s participation, I hereby:
- Release and discharge the City of New York, event organizers, volunteers, and sponsors from any and all liability, claims, or demands for personal injury, property damage, or wrongful death arising from participation in the Event.
- Agree this release applies to any injury, loss, or damage, whether caused by negligence or otherwise.
3. Medical Consent
In the event of an emergency, I authorize the event organizers or their representatives to obtain medical treatment for my child.
I agree to be responsible for any costs associated with such treatment.
4. Photo/Media Release (Optional)
To my child being photographed or recorded during the event.
